Auction Donation Form

Please note, all donations should be delivered to Cascade K-8 Community School at 2800 NE 200th St, Shoreline, WA 98155 between January 15 and March 22, 2019.

If your donation is not a physical item (such as an experience) and you would like us to create a certificate for you, please note that below.

DONORS: Upon receiving confirmation of your donation via email, please print the email and attach to the item for tracking purposes. 

SOLICITORS (Community members procuring donations): If you are completing this form on behalf of a donor, please put the donor's information in "Contact Information" so that they will receive an email receipt. After submitting the form, please print the summary that appears on this site and submit it with the donated item.
